The Dietary Management Package is for those who’ve already completed the Complete Assessment Package in the last 6 months and would like additional support or coaching as they implement their Individual Meal Plan or work on weight loss, or metabolic health remission.
Step-by-Step instructions for Booking Your Appointment:
- Download the Intake and Service Option Form and fill it out on your computer (not by hand, please!)
- Email the completed form, any available recent blood test results and your availability to the email address on the form
- Pay for your services below via credit card (includes $15.00 credit card charge plus GST), or you can send an e-transfer and save the credit card charges
