Meal Plan Package – getting started (for outside Canada)

The Meal Plan Package is an all-inclusive nutritional education package for clients outside of Canada who want to eat healthier and/or lose (or gain) weight. If you haven’t already, please visit the Services tab for details.

For those from Canada, please choose the Complete Assessment Package.

Step-by-Step instructions for Booking Your Appointment:

1. Download the Intake and Service Option Form ( Form for Outside of Canada) to your desktop and fill it out on your computer
2. Email the completed pdf form, any available recent blood test results and your availability to the email address on the form
3. Pay for your services below via credit card (or you can send an e-transfer for the amount below, if you prefer)
