If you do not have any clinical conditions and would like to learn about pricing or get started with services, please download and complete the Intake and Service Option Form, complete the form on your computer, then email it to the address on the form.
If you do have any clinical conditions, I would be happy to see you through my long standing practice (BetterByDesign Nutrition Ltd.) where I offer a wide range of services, including low carb and keto. You can download the Intake and Service Option Form for that division under the Book an Appointment tab on the BBDNutrition website.
PAYMENT OPTIONS
Payment for services can be made by credit card in the secure online Shop, or by sending an e-transfer which saves the credit card charges. I will send you an invoice via email with the information needed to make paying by e-transfer easy. For your security, e-transfers are automatically deposited.
Cash flow can be a challenge. If you would like to use the Flexible Payment Option to pay for services, please be sure to download the Flexible Payment Option Form here and send it to the email address on the form, along with your completed Intake and Service Option form.
For scheduling purposes, prepayment for an appointment is required at the time of booking; however, cancellations or rescheduling is available with 24-hours notice, as well as in the event of an unforeseen emergency.

- Download the Intake and Service Option Form to your desktop, fill it out on your computer (not by hand, please) and select your services.
- Email the completed pdf form and any available recent blood test results to the email address on the form.
- Let us know your availability and book your appointment.
